Community Access

Support Coordination helps participants to understand and implement their NDIS plan, connect with service providers, and access the community. The NDIS offers four levels of Support Coordination, ranging from basic support for those with simple NDIS plans to specialist support for those with very complex needs. The appropriate level of Support Coordination is determined by the participant’s individual needs and circumstances. It is designed to help people with disability achieve their goals, increase their independence, and participate in the community.

 

Support Coordination is an important NDIS support because it helps participants to understand and implement their NDIS plan, connect with service providers, and access the community. Without Support Coordination, participants may struggle to understand their plan and how to access the supports and services they are entitled to under the NDIS. This can lead to frustration and a lack of progress towards achieving their goals.

 

Support Coordination also helps participants to navigate the NDIS system and connect with service providers who can help them achieve their goals. This can be especially important for participants with complex needs, who may require assistance coordinating multiple supports.

There are Four levels of Support Coordination:

Basic

This level of Support Coordination is suitable for participants who have a simple NDIS plan and are able to manage their supports with minimal assistance. It may involve help with a few tasks, such as finding a service provider or understanding how to use a particular support.

Standard

This level of Support Coordination is suitable for participants who have a plan in place and are able to manage their supports with some assistance. It may involve help with several tasks, such as connecting with multiple service providers or coordinating a range of supports.

Enhanced

This level of Support Coordination is suitable for participants who need more help to understand and implement their plan, or who have complex needs and require assistance to connect with multiple service providers. It may involve more frequent contact and support to help the participant manage their plan effectively.

Specialist

This level of Support Coordination is suitable for participants who have very complex needs and require specialist assistance to manage their supports. It may involve support from a specialist Support Coordinator with expertise in a particular area, such as behavioral support or transitioning from institutional settings to the community.
