Support Coordination

Achieving Independence with the Help of Professional Support Coordination

What is NDIS Support Coordination ?

Support Coordination is a support offered through the National Disability Insurance Scheme (NDIS) that aims to assist individuals with disability to understand and access the NDIS, plan and implement their NDIS plan, and connect with other support services in their community. The Support Coordinator works with the individual to identify their strengths, needs, and goals, and helps them to access the necessary services and supports to achieve those goals.

Support Coordination can be provided through various types of support, including face-to-face meetings, phone calls, and online communication. It is designed to be flexible and responsive to the changing needs of the individual. The goal of Support Coordination is to help individuals with disability live more independently and actively participate in their community. Wish Support Group is committed to providing high-quality Support Coordination services to assist individuals with disability in reaching their full potential.

How do I start?
We understand that everyone is different – with different goals and needs.

You may also want to have more control over how you use your plan, and therefore, receive less support. It’s important to know that there are three levels of support coordination that is available for you. These are:
Support connection
This is to help you connect with the services you need to reach your goals.
Support coordination
This support provides you with the skills and knowledge so you can best understand and manage your NDIS plan.
Specialist support coordination
This is for people with higher levels of needs and need extra support to address complex barriers in achieving their NDIS plan.
So, once you know the level of support required, how do you know which support coordinator is right for you? Our team works with you – and we’re well qualified to meet a wide range of needs. That being said, to choose the support coordinator that’s a best fit, here are some questions you should be asking:

Have you previously worked with someone that has similar needs to me?

How would you help me reach my goals?

How much do you know about the services in my local community?

To receive an NDIS plan you will need to attend a planning session either in person or over the phone with the NDIS. This meeting will go over everything from the community supports available in your area, to the equipment you need to take care of yourself at home. You will also discuss the goals you want to achieve and what supports you need.
